to support our work around harm reduction
& community education.
Meet the Harm Reduction Coalition Grantees
In order to select grantees, Harm Reduction Coalition put together a rigorous process to create a request for proposals. The process included five listening sessions across the region with community stakeholders, a session on theory of change and evaluation planning session to create the basis for proposal evaluations and a selection committee to review and score each submission. After the scores were submitted, the whole team gathered together to review all 120 submissions and narrowed it down to 44 organizations. The Harm Reduction Coalition is proud to announce these 44 grantees that represent a total of $5.3 million in local support across Indiana, Kentucky, Tennessee, West Virginia and North Carolina.
The $5.3 million funding represents a commitment to support community partnerships aiming to implement and expand evidence-based, practical strategies in the fight to address the sharp rise in hepatitis C infections. Grantee funding is broken out into two grant types: "provide" grants, which offer new or expanded harm reduction services for people who use drugs, and "educate" grants, which offer new strategies to engage and educate the community on harm reduction.

Indiana
According to HepVu, between 2013 and 2016 there were an estimated 40,200 people living with hepatitis C in Indiana.1
Aliveness Project
Focus Area: Indiana
Grant Amount & Type: $150,000; Provide
Description: The Aliveness Project is a community center for and driven by people living with HIV that facilitates connections to community, offers nutrition and wellness services and links members to resources to help them lead fulfilling and healthy lives. The Aliveness Project will use HepConnect funding to provide a holistic approach to harm reduction and hepatitis C screening and linkage to care services through its CIRCLE (Combining Innovation and Resources for Community Learning and Engagement) Project. The goals of CIRCLE include training staff on scientific advances and harm reduction principles, expanding current screening services to include harm reduction counseling, case management and patient navigation for people who use drugs and piloting peer support interventions to increase the number of appointments and referrals attended. Aspire Indiana Health
Focus Area: Indiana
Grant Amount & Type: $57,041; Provide
Description: Aspire Indiana Health is a fully integrated healthcare provider that offers primary medical and behavioral healthcare, and also addresses social determinants of health like employment and housing. Aspire Indiana Health will use HepConnect funding to support its new mobile HIV/hepatitis C testing and outreach program. The mobile testing clinic will help increase testing, reduce stigma and connect Aspire Indiana Health’s clients with much needed services. Imani & Unidad
Focus Area: Indiana
Grant Amount & Type: $127,894; Provide
Description: Imani & Unidad seeks to help marginalized members of the South Bend and Elkhart communities to improve their social, mental and emotional well-being through counseling, advocacy and education. Imani & Unidad will use HepConnect funding to implement a hepatitis C virus (HCV) care continuum system of services. The goal is to identify undiagnosed individuals, as well as those diagnosed but not in care, and provide successful patient navigation through the continuum from diagnosis to cure. Indiana Recovery Alliance

Grant Amount & Type: $150,000; Provide
Description: The Indiana Recovery Alliance is a statewide grassroots organization dedicated to the implementation of harm reduction interventions, public health strategies, drug policy transformation and justice reform in Indiana and throughout the Midwest. The Indiana Recovery Alliance will use HepConnect funding to continue to operate as a needs-based syringe access program, purchase both sterile supplies and Naloxone and support staff. Description: The Indiana Recovery Alliance is a statewide grassroots organization dedicated to the implementation of harm reduction interventions, public health strategies, drug policy transformation, and justice reform in Indiana and throughout the Midwest. The Never Alone Project

Description: The Never Alone Project is dedicated to providing mutual aid, connection to resources and advocacy and education for people who use drugs, their loved ones and their communities. The Never Alone Project will use HepConnect funding to expand harm reduction outreach services to inner-city Indianapolis, focusing on communities hardest hit by the ongoing war on drugs. In Kentucky, about 42,500 people were living with hepatitis C from 2013-2016.2
Appalachian Regional Healthcare
Focus Area: Kentucky
Grant Amount & Type: $112,500; Provide
Description: Appalachian Regional Healthcare is a not-for-profit health system serving 350,000 residents across Eastern Kentucky and Southern West Virginia. Appalachian Regional Healthcare will use HepConnect funding to expand its outreach to encompass a wide array of services targeted to meet the at-risk population in the Appalachian region service areas. This would include offering individuals equipment such as clean cookers, sterile water and tourniquets; providing peer specialists and education to the user related to harm reduction techniques; enhance the current syringe exchange program and encourage other communities to consider needle exchange programs based on impact and results within affected communities. AVOL Kentucky

Grant Amount & Type: $112,500; Provide
Description: AVOL Kentucky’s mission is to collaborate with communities to end HIV in the Commonwealth. AVOL Kentucky will use HepConnect grant funding to provide additional harm reduction and prevention supplies; increase hepatitis C virus testing outreach, screenings, and connection to health treatment; increase community harm reduction education and awareness; and create and develop an HCV Harm Reduction Peer Advisory Council for Central and Eastern Kentucky. Description: The mission of the Madison County Health Department is to protect, promote and improve the health of the Madison County community. The Madison County Health Department will use HepConnect funding to increase education in the community about substance use disorder and help those living with this disorder better understand safe practices for living with the disease. Funding will also help to establish a local harm reduction coalition to increase collaboration among providers who work with vulnerable populations and expand harm reduction programming in Madison County. Madison County Health Department

Description: The mission of the Madison County Health Department is to protect, promote and improve the health of the Madison County community. The Madison County Health Department will use HepConnect funding to increase participation in its harm reduction syringe exchange program by expanding locations and hours using a mobile unit deployed to multiple locations throughout Madison County. Muhlenberg County Health Department

Description: The Muhlenberg County Health Department’s mission is to prevent illness and injury, promote good health and assure a safe environment to protect and improve the health of citizens. The Muhlenberg County Health Department will use HepConnect funding to expand its syringe program, as well as provide naloxone to first responders, non-injection drug users or concerned family members of individuals dealing with drug addiction. Whitley County Health Department

Description: The Whitley County Health Department strives to deliver high quality, cost-effective healthcare in the community it serves and works to protect the health of the public. The Whitley County Health Department will use HepConnect funding to add a case manager to its harm reduction program that provides vaccinations, HIV and hepatitis C testing, clean syringes, dirty syringe disposal, naloxone, condoms, education about disease prevention and counseling and referral to substance abuse treatment. Norton Healthcare
Focus Area: Kentucky
Grant Amount & Type: $100,000; Educate
Description: Norton Healthcare is a leader serving adult and pediatric patients in Louisville Metro, Southern Indiana, and across the Commonwealth of Kentucky. Norton will use HepConnect funding to support syringe exchange programs and their clients, state health districts, law enforcement and government, local residents, and other stakeholders in a process to collectively determine barriers and best practices within the cultural and legal context of the Commonwealth. University of Louisville Research Foundation

Grant Amount & Type: $100,000; Educate
Description: The University of Louisville is a premier, metropolitan research institution. The University of Louisville will use HepConnect funding to develop a series of educational programming for healthcare providers—primarily pediatricians and nurses—describing harm reduction strategies, principles, and opportunities for incorporation into clinical practice. The educational programming will encourage destigmatizing approaches to the care of people who use drugs, especially women, pregnant women and their families. Center for Prevention Services
Focus Area: North Carolina
Grant Amount & Type: $112,500; Provide
Description: The Center for Prevention Services offers numerous evidence-based drug education programs and serves youth and families of all ages in Mecklenburg, Cabarrus, Rowan, Stanly, Union and Davidson counties. With HepConnect funding, the Center for Prevention Services will continue to provide syringe access to marginalized communities under its Queen City Needle Exchange program, aid mobile exchange services and increase testing and linkage services. Hyde County Health Department

Description: The mission of the Hyde County Health Department is to promote, protect and ensure the health of Hyde County and surrounding areas by providing public health services essential for a safe and healthy community. Description: North Carolina Survivors Union (NCSU) aims to improve the lives of people who have been targeted by the war on drugs by: organizing, growing, and strengthening, community led grassroots groups; educating, mobilizing and advocating for policies and programs grounded in harm reduction, healing and disability justice; providing direct services in areas where high risk communities are denied even the most basic services. Description: North Carolina Survivors Union (NCSU), in partnership with Advocacy House Services, Inc, aim to collaborate to improve the lives of people who have been targeted by the war on drugs by promoting the well-being, health, and dignity of marginalized community members using a social and racial justice lens so that they can live safely, authentically, and free from discrimination regardless of their lifestyle choices, gender identity, or expression. Using a collective voice anchored in science and informed by community leadership, their efforts and programs advance harm reduction techniques and practices that impact our communities through education, training, and advocacy. Description: Steady Collective’s goal is to improve overall community health by reducing the rate of drug overdose and the spread of infectious disease with education, advocacy and direct services. Twin City Harm Reduction Collective

Description: The Twin City Harm Reduction Collective (TCHRC) is a harm reduction organization founded by and for drug users based in Winston-Salem, North Carolina. With HepConnect funding, TCHRC will establish the “Educate, Organize, Mobilize” project in order to build coalitions in surrounding counties that will educate the community on the benefits of syringe service programs and how they can help reduce the risk of HIV, hepatitis C virus, and endocarditis with the goal of building at least one SSP per county. North Carolina AIDS Action Network

Description: The North Carolina AIDS Action Network works to create a North Carolina that is free from stigma and discrimination and provide mutual support for people living with HIV/AIDS in North Carolina through education, advocacy and community. With HepConnect funding, the North Carolina AIDS Action Network will develop a peer education and outreach program specifically aimed at young men who use drugs in order to raise awareness about the HCV epidemic, promote HCV prevention strategies focused on harm reduction and emphasize the disparate impact on people of color in the community. Choice Health Network/Positively Living
Focus Area: Tennessee
Grant Amount & Type: $125,876; Provide
Description: Choice Health Network (CHN) strives to provide a full spectrum of care to the East Tennessee community, from harm reduction and prevention services to case management and medical care for people living with HIV. CHN will use HepConnect funding to bolster its burgeoning harm reduction program and meet community needs for syringes, injection supplies and naloxone. Description: The Next Step Initiative uses a comprehensive approach, including art, movement, mindfulness, poetry and music, to help families who are dealing with the opioid crisis. The Next Step Initiative will use HepConnect funding to support and expand its harm reduction and engagement services. With HepConnect funding, the Next Step Initiative will provide education, direct harm reduction and support services, as well as connect clients to needed services through active case management. Description: The Next Step Initiative uses a comprehensive approach, including art, movement, mindfulness, poetry and music, to help families who are dealing with the opioid crisis. The Next Step Initiative will use HepConnect funding for a multi-purpose space to help cultivate ongoing learning and better health outcomes. With a dedicated space, Next Step Initiative will increase self-esteem and engage people who use drugs by providing access to clean laundry and showers, a computer center and a place for leadership and peer-to-peer opportunities. Street Works
Focus Area: Tennessee
Grant Amount & Type: $150,000; Provide
Description: Street Works is one of Tennessee’s leading HIV service organizations, providing free, confidential HIV testing and supportive services to persons living with HIV disease in the Nashville/Middle Tennessee area since 1997. Street Works will use HepConnect funding to expand and enhance hepatitis-related services for racial/ethnic minority individuals who use drugs. Funding will support the development of a culturally appropriate, trauma-informed and gender-specific continuum of care to meet the needs of individuals who use opiates and other substances and are at high-risk for contracting hepatitis C, including people who inject drugs. Tennessee Recovery Alliance

Grant Amount & Type: $149,639; Provide
Description: Tennessee Recovery Alliance works with other organizations to empower people impacted by substance abuse through mobilization, training and education. Tennessee Recovery Alliance will use HepConnect funding to expand harm reduction services. Efforts will include surveying people who use drugs in Knox County and surrounding counties and assisting with training for wound care for people who use drugs, overdose reversal, safer injection, medication-assisted treatment, and providing naloxone. Description: Tennessee Recovery Alliance works with other organizations to empower people impacted by substance abuse through mobilization, training and education. Partnership to End AIDS Status (PEAS)
Focus Area: Tennessee
Grant Amount & Type: $135,998; Educate
Description: Founded with the goal of eradicating HIV/AIDS and other health disparities in the communities of Memphis, TN, Partnership to End AIDS Status (PEAS) is a grassroots organization serving communities of color to ensure physical, mental and social equality for all residents of the Memphis Transitional Grant Area by providing tools to make healthier decisions. PEAS will use HepConnect funding for peer-led advocacy and mobilization that aims to advance the rights and wellbeing of trans/cis women of color who face disproportionate structural inequities. PEAS will host HIV/hepatitis C/sexually transmitted infection educational workshops throughout the community and provide harm reduction resources during non-traditional service hours.
WeCareTN
Focus Area: Tennessee
Grant Amount & Type: $150,000; Educate
Description: WeCareTN works to support and inspire trans women of color in Tennessee. WeCareTN will use HepConnect funding to expand its Transpro program, a job readiness course for current or former sex workers led by current or former sex workers. WeCareTN will integrate harm reduction into the course to create a more holistic approach to improving the lives of trans women, building self-esteem, and cultivating leadership. Cabell-Huntington Health Department
Focus Area: West Virginia
Grant Amount & Type: $127,500; Provide
Description: Cabell-Huntington Health Department (CHHD) works to improve the health of people in West Virginia through their clinical, environmental, epidemiology and health and wellness services. With HepConnect funding, CHHD will expand the capacity of its harm reduction programs by providing clean syringes. Opioid Response Innovation Initiative
Focus Area: West Virginia
Grant Amount & Type: $126,830; Provide
Description: The Opioid Response Innovation Initiative is working on the front lines of the opioid crisis to help organizations save more lives through human-centered design, the use of technology, and innovative approaches. With HepConnect funding, the Opioid Response Innovation Initiative will create a new peer support network to help prevent hepatitis C transmission and help individuals accessing recovery services. Funding will also support the work of two peer coaches who will be responsible for distributing narcan to county health departments. Milan Puskar Health Right/LIGHT Project
Focus Area: West Virginia
Grant Amount & Type: $127,500; Provide
Description: Milan Puskar Health Right (MPHR) is a free clinic for West Virginia's residents who are low-income, uninsured, underinsured or have Medicaid. With HepConnect funding, Milan Puskar Health Right will expand hepatitis C prevention services, mentor and train communities in harm reduction techniques and help people navigate the healthcare system. Description: The Virginia Harm Reduction Coalition (VHRC) provides harm reduction resources and education to rural populations impacted by hepatitis C. With HepConnect funding, VHRC will expand its harm reduction education programs, increase its networks and service delivery capabilities, and shift material resources to rural communities.

Description: The Virginia Harm Reduction Coalition (VHRC) provides harm reduction resources and education to rural populations impacted by hepatitis C. With HepConnect funding, VHRC will expand its harm reduction education programs, increase its networks and service delivery capabilities, and shift material resources to rural communities.
Cabin Creek Health Systems
Focus Area: West Virginia
Grant Amount & Type: $127,500; Educate
Description: Cabin Creek Health Systems (CCHS) is a network of community health centers in Kanawha County that offers MAT and infectious disease screening and treatment as part of its primary care program. Coalition funding will be used to develop a community dashboard to measure the extent of the epidemic and effectiveness of interventions. Funding will also be used to reduce the harm of addiction through a community education campaign and by utilizing CCHS' school based centers to identify and support children who are at risk for substance use disorders. Description: The Community Education Group (CEG) develops creative and culturally appropriate programs to stop the spread of HIV and Hep C. CEG works to include the communities it serves, including people of color, transgender Americans, sex workers and ex-offenders. With HepConnect funding, the CEG will expand harm reduction services to include safe syringe programs, area cleans and linkages to care. These programs will increase public awareness and reduce community vulnerability to hepatitis C and HIV acquisition.
